The Young Families Program is a scattered site housing program for young families within Hennepin County. This program provides housing, supportive services and early childhood programming to help young families transition out of homelessness.

 

The program helps young families experiencing homelessness transition quickly from the streets and shelters to stable housing. The program provides family-focused services, activities and case management support.

Families Rapid Re-Housing (RRH) and Housing Support Programs serves individuals ages 18-24 who are pregnant or parenting and facing housing instability/homelessness.

Participants must:

Be referred by Hennepin County Coordinated Entry

Be able to provide documentation of homelessness in Hennepin County on the day of intake with a letter from the emergency shelter that one is residing at or from an out reach worker indicating the person is living in a place not meant for human habitation.

Be capable of living independently, be willing to participate in Rapid Re-Housing or Housing Support Program services.
