Minor Connect is a collaboration with the Minnesota Department of Human Services, Hennepin County Human Services, child welfare and nonprofit agencies including The Link, YMCA, and The Youth Law Project.
Minor Connect aims to create a dynamic response to assist youth ages 15-17 who are at risk of homelessness, experiencing homelessness, or whose housing situation creates the potential for significant harm.
The program also provides trauma informed supportive services for youth that include but are not limited to case management, life skills education, assistance with education and employment, connections to mental health and chemical dependency treatment, connections to health care services and leadership development opportunities.
Eligibility for Minor Connect:
Minor Connect serves youth ages 15-17 who are experiencing homelessness or housing instability.
Must be referred through Hennepin County Child Protection Services by calling 612-348-3552. Ask to be referred to the Minor Connect Pilot Program.
