About Community Resource Navigator Program

The Community Resource Navigator Program serves youth between the ages of 13 and 24 who have been involved in crime. Youth work one-on-one with case managers to create sustainable and achievable goals around education, healthy relationships, restorative justice, employment, obtaining vital documents and more. Case managers also provide connections to appropriate community resources including housing referrals, parenting support, and career development programs.

“While in the program I obtained my state ID, social security card, medical insurance, SNAP benefits, connections to parenting support and an apartment for myself and my family.”

Referral Process

Young people ages 13-24 are referred to the Community Resource Navigator program through the Hennepin County Department of Corrections.
