About Housing First

The Housing First Program offers 30 units of scattered-site supportive housing in Hennepin County for youth experiencing homelessness that also have a mental health and/or chemical dependency diagnosis or a physical disability.

In addition to housing, The Link also provides trauma informed and strength based supportive services for youth that include but are not limited to case management, life skills education, assistance with education and employment, connections to mental health and chemical dependency treatment, connections to health care services, support groups, positive youth development activities and leadership development opportunities.

Eligibility

Housing First serves homeless, singles who have a mental health and/or chemical dependency diagnosis or a physical disability ages 18-24.

Participants must:

  • Qualify as long-term homeless: Long Term Homeless Definition
  • Have long term disability that significantly impairs employment
  • Be eligible for General Assistance
  • Be between the ages of 18-22 at the time of application

Referral Process

Youth are referred to the program through the Hennepin County Coordinated Entry system. Coordinated Entry Assessments and updates can be made at shelters, drop-in centers or by connecting with an outreach worker. To connect with The Link’s Street Outreach Team, please call or text: (612) 718-1701.
