About Young Families

The Young Families Program is a scattered site housing program for young families within Hennepin County. This program provides housing, supportive services and early childhood programming to help young families transition out of homelessness.

Services

Case managers assist parenting youth in transitioning out of homelessness and into safe, supportive, and sustainable housing which matches their family’s needs. Youth work with a case manager to craft individualized goals that focus on independent living and tenancy skills, employment, parenting, and early childhood development. Programming includes regularly scheduled parent support groups and family friendly activities.   

Referral Process

Families are referred to the program through Hennepin County Coordinated Entry. To receive a family coordinated entry assessment, contact Agate Housing and Services at 612-361-5439.
